This is a useful little sheet you can print out and keep in your pocket book, which you can use to give a full ATMIST style handover to a trauma or medical team and not forget anything.
It prints with four on a page so you can cut them down to A6.
All you need to do is sketch on your patient details and read them back from top to bottom during handover. You can then give the sheet to the continuing clinician for their reference, or put it into confidential waste.
